Luteinising Hormone (LH)
Luteinising Hormone (LH) is a hormone produced by the pituitary gland that plays a crucial role in regulating the menstrual cycle and ovulation. In premenopausal women, LH levels fluctuate throughout the cycle and trigger the release of an egg from the ovaries. In those undergoing HRT or transitioning into menopause, LH levels typically rise due to reduced ovarian function. This marker is included in the test to help assess the status of the reproductive system and how it is responding to or requiring hormone supplementation. In gender-affirming HRT, LH can also be used to monitor suppression or stimulation of gonadal activity.

Follicle Stimulating Hormone (FSH)
Follicle Stimulating Hormone works in tandem with LH and is responsible for stimulating the growth of ovarian follicles in the early part of the menstrual cycle. Like LH, FSH levels tend to rise as the body moves toward menopause or when ovarian function declines. Measuring FSH helps determine the need for HRT or evaluate whether hormone therapy is adequately supporting hormonal balance. Persistently elevated FSH may indicate insufficient oestrogen levels, while suppressed levels could reflect effective hormonal feedback from HRT medications.

Oestradiol (Oestrogen)
Oestradiol is the main form of oestrogen in the body and a central focus of most hormone replacement therapies. Oestradiol supports a wide range of physiological functions, including bone density, cardiovascular health, skin elasticity, mood regulation, and sexual function. Low levels are associated with menopausal symptoms such as hot flushes, night sweats, vaginal dryness, and mood changes. This biomarker is included to ensure that HRT is maintaining oestradiol within a healthy range—high enough to relieve symptoms but low enough to avoid unnecessary risk, particularly in breast and endometrial tissues.

Progesterone
Progesterone is another key reproductive hormone that works in balance with oestrogen to regulate the menstrual cycle and protect the uterine lining. In women with a uterus undergoing oestrogen therapy, progesterone is often prescribed to prevent endometrial overgrowth, which can lead to hyperplasia or even cancer. Measuring progesterone helps evaluate whether the body is producing enough naturally or if supplementation is needed. It also helps assess ovulatory function in premenopausal women and supports the monitoring of hormone levels in gender-affirming care or fertility treatment settings.
