Testosterone
Testosterone is a key hormone in male physiology, responsible for muscle growth, libido, energy levels, and overall anabolic activity. This test measures the total amount of testosterone circulating in the bloodstream, both bound and unbound. It helps assess whether testosterone levels are elevated due to exogenous (external) supplementation or suppressed due to feedback inhibition of natural hormone production. Free Testosterone

Sex Hormone-Binding Globulin (SHBG) is a protein that binds to sex hormones, including testosterone and oestradiol, regulating their availability to tissues. PEDs can significantly alter SHBG levels—often reducing it with anabolic steroid use, thereby increasing free testosterone. Monitoring SHBG helps interpret both total and free testosterone values and gives insights into how PEDs are influencing hormone transport and availability.

Oestradiol
Oestradiol is a form of oestrogen that is naturally produced in men through the aromatisation of testosterone. While oestradiol plays roles in bone health and mood regulation, excessive conversion—often caused by high testosterone doses—can lead to undesirable effects such as gynaecomastia (breast tissue development), water retention, and fat gain. This marker is essential in balancing the androgen-oestrogen ratio in PED users and guiding the need for aromatase inhibitors.

Prolactin
Prolactin is a hormone involved in reproductive function and is typically elevated when oestrogen levels are high or when anabolic substances disrupt the hypothalamic-pituitary axis. Elevated prolactin levels can lead to side effects such as low libido, erectile dysfunction, or nipple sensitivity. Tracking prolactin is useful in identifying hormonal imbalances linked to PED use and may flag the use of compounds like nandrolone or trenbolone, which are known to influence prolactin pathways.

Cortisol
Cortisol is the primary stress hormone and has a catabolic effect—breaking down tissue rather than building it. Monitoring cortisol is important in the context of PEDs because chronic suppression (or, in some cases, elevation) of cortisol can affect immune function, recovery, sleep quality, and mood. Some users intentionally suppress cortisol to reduce muscle breakdown, but this can carry long-term risks. Measuring cortisol helps balance anabolic and catabolic activity.

Albumin
Albumin is a major blood protein produced by the liver. It plays a role in maintaining fluid balance and transports hormones, including testosterone. It is also used in calculating bioavailable testosterone. Low albumin may indicate liver stress or malnutrition—both of which can be influenced by PED use. Monitoring albumin helps evaluate liver health and protein metabolism.

Globulin
Globulin is another protein group that includes SHBG and plays a role in transporting hormones through the bloodstream. Abnormal globulin levels can reflect changes in liver function, chronic inflammation, or hormonal imbalance—all of which are relevant in long-term or high-dose PED use. It contributes to the overall picture of how hormones are being distributed and utilised.

Total Protein
Total Protein is the sum of albumin and globulin in the blood. It reflects general nutritional status and liver function. PED use, particularly substances that impact the liver, can alter protein synthesis. Low or high protein levels can signal underlying conditions that may be worsened by PEDs, including liver damage or dehydration from diuretics and cutting agents.

Platelet Count
Platelet Count is part of the complete blood count and measures the number of platelets, which are critical for blood clotting. Elevated platelet levels may be seen with certain anabolic agents and can increase the risk of clotting events. Monitoring platelet count is essential for assessing cardiovascular safety, especially when PEDs elevate haematocrit or red blood cell mass.

Mean Platelet Volume (MPV)
Mean Platelet Volume (MPV) assesses the average size of platelets, providing insight into platelet production and activation. Larger platelets are often more reactive and can indicate increased clotting risk. As PEDs can stimulate red cell and platelet production, this marker is important in evaluating potential thrombotic complications.

Haematocrit
Haematocrit measures the proportion of red blood cells in your blood. Elevated haematocrit is a common side effect of PEDs like testosterone and erythropoietin, which stimulate red blood cell production. While this can enhance endurance and oxygen delivery, excessively high haematocrit thickens the blood and increases the risk of stroke, heart attack, or deep vein thrombosis. Regular monitoring is essential for cardiovascular safety.

Haemoglobin
Haemoglobin is the oxygen-carrying protein in red blood cells. Like haematocrit, haemoglobin levels often rise with anabolic steroid use. High haemoglobin can indicate improved oxygen transport but also poses a risk for blood viscosity and clot formation. It is a key marker for detecting overproduction of red blood cells, which may require therapeutic blood donation or adjustment of PED dosage.

Full Blood Count (FBC)
Full Blood Count (FBC) provides a broad overview of blood health, including white blood cell count, red blood cell count, haemoglobin, haematocrit, and platelet levels. It helps detect infections, inflammation, anaemia, or blood cell abnormalities. In the context of PEDs, it gives an essential snapshot of how these substances may be affecting overall haematological balance and immune function.

Prostate-Specific Antigen (PSA)
Prostate-Specific Antigen (PSA) is a protein produced by the prostate gland and is used as a screening tool for prostate enlargement or cancer. Elevated PSA levels can be a concern in men using high doses of androgens, which may stimulate prostate tissue growth. This marker is especially important for male users over 40 or those with a family history of prostate issues, offering early detection of potential prostate-related side effects.
